There has been a tremendous response to registration for the new Government of Jamaica (GOJ) Health Card.  Thirty thousand registration forms were issued and at the end of the first day of registration most public health facilities reported that their batch of registration forms was fully utilized. Additional forms were dispatched today, while more will be distributed to the Regional Health Authorities by tomorrow.
 
The GOJ Health Card was launched on Friday, January 31, 2014 and registration began on February 3, 2014 in all public health facilities, the Ministry of Health’s Head office and the National Health Fund.
 
In order to register persons will be required to provide their name, address, phone number, Tax Registration Number (TRN) and in the case of PATH beneficiaries, their PATH numbers.  Once filled out the card can be left at any public health facility, the Ministry of Health’s head office, any office of the Regional Health Authority, any NHF help desk and the NHF head office. Persons should ensure that the address given is correct as the card will be mailed.
 
The use of the card will begin in all public health facilities on March 3, 2014.
